Duplicate Tax Statement Request
Requests for Tax Year 2020 forms will be accepted and processed as of the following dates:
- W-2 reprints: February 15
- Fellowship Letters: March 1
- 1042-S: March 15
Requested forms will be encrypted with security protection and emailed to the email address you provide below.
Currently Active Employees
Tax statements for 2018 to present are available in the My UW System portal. You can download, view and print duplicate tax statements via the Tax Statements tab in the Payroll Information module. If you require a tax document prior to 2018, follow the request process below, and note the fee(s) required.
Inactive or Former Employees
All duplicate requests must follow the request process below.
1042-S forms will be available by early March. If you have a Glacier account and did not opt out of electronic delivery, you will receive an email with retrieval instructions. Otherwise, your form will be mailed via U.S. mail.
Only certain individuals receive Form 1042-S; those that had a tax treaty exemption applied to their income, and those who received a scholarship or fellowship through payroll before passing the IRS substantial presence test.
Before requesting a duplicate 1042-S, make sure you were eligible to receive one. Compare your total taxable income for the year to the amounts reported on Form W-2 and the fellowship letter. Each dollar of income or stipend will only be reported on one form/letter.
- Requests for the previous calendar year are processed at no charge.
- Requests for duplicates from 2-5 years ago are $5.00 per form, per year.
- Requests for duplicates from before 6 or more years ago are $10.00 per form, per year.
Submit a check, cashier's check or money order payable to the University of Wisconsin. Duplicates will not be processed without payment. Please allow 10 days for processing.
