All requests must follow the process below.

1042-S Requests

This form only apply to people who:

had a tax treaty exemption applied to their incomes OR
received a scholarship or fellowship through payroll before passing the IRS substantial presence test

Before requesting a duplicate 1042-S, make sure you were eligible to receive one.

Fellowship / Scholarship Letter

Reports non-service scholarships / fellowships paid to resident aliens, permanent residents or US persons. For Resident Aliens and/or Permanent Residents, each dollar of income is only reports on one form / letter based on your residency status.

1098-T (Tuition Statement)

Please contact your campus bursar's office for a copy of this form.

Documents from 2017 and earlier are $10 per form, per year.
